Eric Schlosser's Command and Control [ http://www.nytimes.com/2013/09/15/books ... d=all&_r=1 ] documented many close calls involving U.S. nuclear weapons nearly inadvertently detonating, including the 1980 ignition of a missile's liquid fuel, at a silo in Damascus, Arkansas that was initiated when a repairman dropped a socket wrench.

Now, Nukewatch Wisconsin has published an update on the remaining active silos, entitled Nuclear Heartland, Revised: A Guide to the 450 Land-Based Missiles of the United States.
